REQUIRED INFORMATION:
- Name of company
- Billing and shipping address
- Contact name & title
- Email address & phone number
- URL of vacation rental home, inn, B & B, or hotel on a published site.
VERIFYING DOCUMENTS
Please send a copy of the following verifying documents to complete your application. Valid documents are required to qualify. and they should be one of the following:
- A current business or state professional license.
- Resale or sales tax certificate - mandatory if claiming tax-exempt status.*
- Link to vacation home rental listing on published rental site.
Other supporting documents may include:
- Proof of ASID or AIA membership.
- Interior design certification (NCIDQ, CCIDC).
*TAX EXEMPTION RESALE CERTIFICATION
If your company has a resale certificate to receive tax exemption status, you must provide a valid and active resale certificate issued by your state whereby the name of purchaser on the certificate matches the company name on this application. A business tax ID or retail certificate is not a valid document for tax exemption.
